LGBTQ+ Advance Care Planning Toolkit

LGBTQ+ people can face particular obstacles in legal and healthcare systems. SAGE and its National Resource Center on LGBTQ+ Aging have published the LGBTQ+ Advance Care Planning Toolkit, a free, step-by-step guide to preparing for future healthcare needs and end-of-life decisions.

The toolkit addresses these challenges across four stages: planning while well, preparing for serious illness, end-of-life planning and honoring life. It explains health care proxies, living wills, financial and funeral planning, palliative and hospice care, and finding LGBTQ+-affirming providers. It also includes worksheets, a sample letter to caregivers and a checklist for an “In Case of Emergency” folder.

CNP shares this resource as part of its commitment to generous hospitality and to the dignity of all God’s people at every stage of life.
